Abstract
The First 24hrs Matter initiative proactively addressed inefficient discharge planning to improve patient throughput. By implementing early Case Management evaluations, standardized workflows, and promoting 2 PM discharges, the project successfully increased 24-hour assessment completion, boosted early discharges, and reduced both average length of stay and excess days.
