Abstract
Building on a successful "Call Don't Fall" initiative for alert patients, NSUH identified sundowning-related confusion as the cause of remaining falls. In 2025, they launched the "Twilight Tracker" project to proactively manage fall risk in these patients. This involves staff completing a daily rounding form during evening and night shifts to identify at-risk individuals, initiate specific interventions, and ensure enhanced rounding. Since its launch on July 15, 2025, the unit has completed 68 trackers, achieved 100% increased bed/chair alarm utilization, and recorded **no falls** in the sundowning patient population for 3.5 months, thus creating a comprehensive fall prevention strategy.
