Abstract
Recognizing that frequent staff transitions and inadequate handoff tools disrupted care, the project restructured relief schedules and introduced the "SWITCH Handoff Tool." Preliminary results indicate that 40% of staff felt more prepared after using the new tool, suggesting improved RN-to-RN communication. The initiative is in its early stages, with plans for ongoing education and optimization to further strengthen patient safety protocols.
